A low medication error rate is desirable. Therefore, we expect the organization to have done something positive. The best answer in this case is A.

The phrasing of the question stem can be improved by changing “… may be…” to something like “… is most likely…”. “May be” allows several possible answers for this question. For example, reluctance on the part of staff to report may also result in a low observed rate of medication error. Nevertheless, for questions with more than 1 viable answer, you are expected to give the best answer, i.e. answer option A for this question.
